Click on the Customize layout button to open the Customization window, where all available fields of the current table are listed.
The Root section contains all fields already present in the form view.
The hidden fields section contains all fields that are not yet in the form view.

Drag the fields, from the Hidden Items list, to the form.
To remove a field, drag the field from Root list to the Hidden Items list.

Use Empty space items to control the size of the fields in the form.

Click on the Apply button to save the changes made in the table view settings and click on the Close button to close the window.
